Life Stage Transitions
We know that transitioning to a new stage of life is a challenging time for anyone, and if you have a disability, it can be much harder. At PrimeCare we excel at helping NDIS participants through life stage transitions with a planned approach. There are many transition points throughout a lifetime - starting school, leaving home, getting that first job, or retiring from work, for example. With proper planning and networking, we can support you through life's transitions and on to achieving your short term or long term goals.

Personal Care/ Activities
Personal care supports relates to assistance with, or supervision of, personal tasks of daily life.
Our team go above and beyond to make sure that help is given to individuals in a variety of settings, for example, a participant living alone in their own home, living with family or other people, when undertaking social, recreational, education or employment activities or during holidays away.
Our services include
- Personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
- Toileting, bladder and bowel management and menstrual care;
- Eating and drinking;
- Attending appointments
- Mobility and transferring, for example moving in and out of bed and on or off the toilet.
- Application of splints, basic first aid due to injuries sustained as a result of a participant’s-disability.
